Job Title: Medical Social Worker

Job Summary:
We are looking for compassionate and qualified Medical Social Workers to join healthcare teams across Canada. In this role, you will support patients and their families through emotional, social, and practical challenges associated with illness, hospitalization, and long-term care.
Key Responsibilities:
- Assess the psychosocial needs of patients and families in medical settings
- Provide counseling and emotional support to patients dealing with serious or chronic illness
- Assist patients in navigating healthcare systems and accessing services
- Develop individualized discharge and care plans in collaboration with the medical team
- Connect patients with community resources, housing, financial aid, and rehabilitation services
- Advocate for patient rights and facilitate communication between patients, families, and care providers
- Support crisis intervention and palliative care planning
- Maintain accurate documentation and case notes in line with regulatory standards
Qualifications:
- Bachelor’s or Master’s Degree in Social Work (MSW preferred)
- Licensed or eligible for licensure in Canada (e.g., registration with a provincial social work college)
- 1–2 years of experience in a hospital, clinic, or community health setting
- Strong understanding of medical terminology and healthcare systems
- Excellent communication, assessment, and crisis management skills
- Ability to work collaboratively in multidisciplinary teams
- English or French language proficiency (based on location)
Salary Range:
CAD $55,000 – $75,000 per year (varies by province and experience)
